Three Videos - What Can Go Wrong - Wyoming Accident Examinations

Three videos showing variety of accidents and reporting on what went wrong.

This week we feature three videos from Wyoming OSHA. Using animation they show actual fatal accident situations, and provide an analysis of what went wrong.


Slips, trips and falls are a leading cause of workplace injuries and fatalities. Not following established procedures may seem like a way to get a job done quicker, but it can lead to accident and fatalities.



Lockout/Tagout Failure: a high pressure and temperature steam release during engine room repair results in a fatality.



A worker needed a grinding wheel, but none was available. So, to get the job done, he decided to make a grinding wheel. In this accident equipment was not used properly. The result was a fatality when the grinding wheel exploded.
