Facility Fires result in Inspection, Fines

MENOMONEE FALLS, Wisconsin, $132,800 in fines – A complaint alleging three fires at Quest Specialty Coatings resulted in an inspection, which unearthed eight safety violations.
Quest failed to address the potential for static accumulation and discharge, and did not train employees in the equipment processes and emergency shutdown procedures for the machines they were operating. They also did not ensure that procedures to maintain mechanical integrity were performed, such as inspecting valves and pipes. In addition, they did not inspect procedures and train workers on the hazards of servicing and maintenance, resulting in five serious violations.  >> Full story

OSHA cites Central Transport Again for Forklift, Fall Hazards

HILLSIDE, Illinois, $145,420 in fines – OSHA inspected Central Transport LLC due to a complaint and cited them for 16 safety and health violations. The inspection found repeat and willful violations that involved defective powered industrial vehicles and lack of fall protection.

Central Transport was cited in 2009, 2010, 2013 and in September 2014 for similar violations.  >> Full story
